General Disclosures. Thank you for taking the time to complete this study today. We are conducting a market research study to understand your opinions about Multiple Myeloma. This study should take approximately [30] minutes to complete. Please answer all of the questions to the best of your ability.

ZS, the firm conducting this research, is an independent market research agency and consultancy. The research is sponsored by a life sciences company. This study will require participation virtually that will last for 30 minutes. You will need access to a computer, internet connection and web browser.

Compensation for participation in the study is the amount specified in your invitation email. This compensation is not intended to reward you for use of any products which may be mentioned in the study or induce you to use any such products, but rather to understand your views.

You may withdraw from the market research at any time.

Adverse Events. Different patients sometimes respond in different ways to the same medicine, and some side effects may not be found until many people have used a medicine over a period of time. If you mention a side effect related to a medicine made by the sponsor of the study in your survey responses, we must pass that information on to the sponsor so that they can learn more about the safety of their own medicines. Your anonymity will be protected.

Data Protection. We need your consent for us to collect and use any information about you, which may include your name, your contact information, some information about your health, and your opinions. We will only collect or process data that is necessary to conduct the study, and not all aforementioned categories will be applicable to all studies. To the extent data about you is collected or processed in conjunction with the market research study, it may be transferred to third parties assisting with the study, such as a survey programmer, and will also be stored in the United States. In such cases, the necessary measures will be taken to ensure the safety of your data in accordance with applicable data protection laws.
